New clients are encouraged to log in to our client portal and complete the Developmental History prior to the first session, as this will allow us to spend more time discussing your specific concerns.

If you would like our clinicians to communicate with other professionals, please complete a Release of Information for each organization and include contact information. These forms are also available through the client portal.

If you would like to review the terms of our Service Agreement or our Notice of Privacy Practices prior to setting up an appointment, copies are provided to the right.

Newly scheduled clients will be sent an invitation to access our client portal. Once enrolled, you can access the portal at any time using the following link:
