FEES

We charge a flat fee of $3500 for comprehensive psycho-educational assessments.  This fee includes the initial consultation, all testing (typically scheduled over two days), one in-person feedback session, and a comprehensive written report.  Because of the large amount of time set aside for each client, we require a nonrefundable deposit of $500 in order to reserve testing dates.  We ask that a signed copy of the Service Agreement and a check for the deposit be sent to our office within one week of scheduling in order to hold your testing dates.

 

We are available for consultation and briefer assessment when indicated; please contact us to inquire about fees for other services.  Our typical hourly rate for additional services is $200, though this can vary based on the service being provided.

INSURANCE

Forward Thinking Assessment LLC operates on a fee-for-service basis and accept checks, all major credit cards, and cash.  Because our clinicians function as out-of-network providers, we are not members of any managed care plans and do not accept insurance as a form of payment. However, we will provide you with detailed receipts including all the necessary information should you choose to submit to your insurance company for reimbursement.

 

If you are interested in submitting for reimbursement, we recommend that you contact your insurance company to request information about out-of-network benefits for psychological consultation and testing prior to scheduling an appointment.  We suggest asking whether pre-authorization is required specifically for psychological testing, and what is required to obtain authorization. If pre-authorization is necessary, we are happy to work with you to complete the necessary paperwork prior to beginning an evaluation.  However, it is the responsibility of each client to be aware of your insurance benefits, to provide us with any forms that need to be completed, and to communicate directly with your insurance company.  Please understand that we cannot guarantee any amount of reimbursement, as the final decision rests entirely with your insurance company.

  90791 Diagnostic Interview/Intake Session (please note that the modifier 02 will be added for telehealth sessions)

 

  96136 and 96137 Psychological Test Administration and Scoring 

 

  96130 and 96131 Psychological Testing Evaluation Services (Includes Feedback Session)

Your insurance company may ask you for specific diagnostic codes; we would be happy to discuss these with you. If you have additional questions, please feel free to call us and we can give you more information about the process.

 

CANCELLATION

Because your appointment time is reserved specifically for you, we reserve the right to charge a fee in the event of short-notice cancellations or cancellations without rescheduling.  Our cancellation policy is outlined in more detail in the Service Agreement.
